OCMC’s home and community based healthcare division provides appropriate and effective physician directed care to individuals in their own home. Home care provides relief for family members who give continuous care to patients with long-term illnesses.
Services provided include:
- Dietary Counseling
- Extended Care Services
- Home Health Aides
- Medical Social Services
- Nursing Care
- Physical & Occupational Therapy
- Speech Therapy
Dietary Counseling
Dietary Counseling is available for individuals whose medical condition warrants a special diet. Following a physician referral, the dietitian assesses the patients current eating habits and interprets the physicians order into a workable meal plan. The dietitian offers advice to caregivers on shopping for and preparing proper nutritional choices for the patient.
Extended Care Services
Extended Care is for the patient who requires six or more hours of continuous home health service in a 24-hour period. Extended care is staffed by Registered Nurses, Licensed Practical Nurses and Certified Nursing Assistants. All nursing care is provided under the guidance of the patient’s physician. Patients requiring the following services are eligible:
- Hospice, Rehabilitation, Transplant, etc.
- Oxygen Supplementation
- Parenteral Nutrition
- Peritoneal Dialysis
- Prolonged IV Drug Therapy
- Tube Feeding (gastrostomy, naso or gastric feeding)
- Ventilator
Referrals for extended care are made by the patient’s physician. When Doctors Home Care receives the referral, a Registered Nurse will go to the patient’s home (or the hospital) for an evaluation. Once the in-home services begin the supervisor and nursing team work with the physician to develop a plan of treatment, to review a medication regimen and to conduct patient evaluations in order to ensure the best quality of care for each extended care patient.
There are several payment options for extended care services:

- Private Insurance
- Private Pay (personal check, Visa, MasterCard, Discover)
Note: Extended Care Services ARE NOT covered by Medicare.
Home Health Aides
Home Health Aides are Certified Nursing Assistants who provide help to individuals requiring assistance with daily living activities. Following a physician referral, a registered nurse assesses the patients needs and develops a plan of care that is carried out by an aide. Services provide include:
- Assistance with Ambulation
- Assistance with Bathing
- Catheter Care
- Checking of Vital Signs
- Personal Care
- Preparation of Light Meals
Medical Social Services
Medical Social Services are available for individuals suffering from psychological problems. Social services must be prescribed by a physician and must be included in the physician plan of care. Visits are based on the patients needs:
- Assessment of Emotional Factors
- Assessment of Social Factors
- Counseling for Long Range Planning
- Decision Making and Community Resource Planning
Medical Social Services are classified as secondary services by Medicare. They can only be initiated of a patient requires and is receiving one of these primary services: Skilled Nursing, Physical Therapy or Speech Therapy.
Payment options include:
- Medicare
- Private Insurance
- Private Pay (personal check, Visa, MaterCard, Discover)
These services are generally not covered by Medicaid.
Nursing Care
Nursing Care, Private Duty Nursing)
Nursing Care Services are provided by Registered Nurses, Licensed Practical Nurses and Certified Nursing Assistants. All nursing care is provided under the guidance of the patient’s physician.
Referrals for Nursing Care are made by the patient’s physician. When Doctors Home Care receives the referral, a Registered Nurse Supervisor will go to the patient’s home (or the hospital) for an evaluation and to begin the admission process. Scheduling of in-home services can then begin. The RN Supervisor and nursing team work with the physician to develop a plan of treatment and to conduct patient evaluations in order to ensure the best quality of care for each patient.

Physical & Occupational Therapy
Physical and Occupational Therapy services are provided to help restore a patient’s functions after an illness or injury. The therapists work closely with doctors, nurses, speech-language pathologists and other members of the health care team.
Individuals who may qualify for services are:
- Individuals with work related injuries
- Individuals with arthritis, multiple sclerosis and other serious illnesses
- Individuals who have suffered a stroke or heart attack
- Individuals with broken bones or other injuries resulting from falls or accidents
- Individuals with burns, amputations or spinal cord injuries
- Individuals with mental health problems
Referrals for therapy services are made by the patient’s physician. When Doctors Home Care receives a referral, the patient will be evaluated. Scheduling of in-home services will then begin. The RN Supervisor and nursing team work with the physician to develop a plan of treatment and to conduct patient evaluations in order to ensure the best quality of care for each patient.
