Peri Natal Home Care
Peri Natal Home Care Services are provided to pregnant home bound women. Referrals for Peri Natal Home Care are made by the patient's physician. When Doctors Home Care receives the referral, a Registered Nurse Supervisor will go to the patient's home for an evaluation. Scheduling of in-home services will then begin. The RN supervisor and nursing team work with the physician to develop a plan of treatment and to conduct patient evaluations in order to ensure the best quality of care for each patient.

Personal Care Services
Personal Care Services are defined as medically necessary tasks pertaining to a persons functional abilities which enable him/her to remain at home. Personal Care Services are available to individuals requiring assistance with at least two (2) activities of daily living, i/e., bathing, meal preparation, etc. Services include:

Referrals may be made by family, friends, a community agency or the patient. A physicians consent is required, however, before personal care can begin. When a referral is received, a Registered Nurse is sent to the patient's home to assess the patient's needs. If it is determined that all guidelines are met, a plan of care is sent to the physician for review and approval.Visits are then arranged as needed and the physician is kept up to date on the patient through progress reports.

Physical & Occupational
Therapy
Physical and Occupational Therapy services are provided to help restore a patient's functions after an illness or injury. The therapists work closely with doctors, nurses, speech-language pathologists and other members of the health care team. Individuals who may qualify for services are:

Speech Therapy/Early
Intervention
Speech Language Pathologists work with individuals who have language delays and speech problems. They provide treatment for stuttering, voice and articulation problems and work with stroke victims and patients with Alzheimer's disease to help them regain the use of language, help swallowing mechanisms function more appropriately and provide aid with communication skills.Speech-language, dysphagia, aphasia and oral motor therapies are provided for patients of all ages in the home. Our Early Intervention program offers a variety of of speech-language therapy services for infants and toddlers aged birth-3 years.
