Home Health Aides
Home Health Aides are Certified Nursing Assistants who provide help to individuals requiring assistance with daily living activities. Following a physician referral, a registered nurse assesses the patients needs and develops a plan of care that is carried out by an aide. Services provided include:

Home Delivered Meals
The Meal Mobile Provides nutritionally balanced meals to individuals who are unable to shop for themselves or who are on restricted diets and need help with meal planning. Anyone is eligible for the service. The Meal Mobile delivers 7 heat and serve meals per week to a subscribers home. There are a variety of meal choices and all meals are complimented with a rice or potato side dish, vegetable, roll, desert and beverage. For more information please call:

Your delivery will begin the following week. Meals will be delivered 7 dinners at one time.

Hospice Services
Hospice is a concept of supportive care for terminally ill patients and their families. Hospice offers physical (pain control), emotional, spiritual and social support services as needed. The program strives to allow patients to remain in their own home and be active for as long as possible.
Patients may be referred to hospice by physicians, families, friends, hospital discharge planners and other community agencies. A patient is admitted to the hospice program with the consent and participation of the patient's primary physician. The patient must be in the advanced stages of a life threatening illness with a life expectancy of six months or less. A primary care giver should be available in the home to be trained and be responsible for coordinating care of the patient. For more information or to make a referral please call us at:
Medical Social Services
Medical Social Services are available for individuals suffering from psychological problems. Social services must be prescribed by a physician and must be included in the physicians plan of care. Visits are based on the patients needs and other

Medical Social Services are classified as secondary services by Medicare. They can only be initiated if a patient requires and is receiving one of these primary services: Skilled Nursing, Physical Therapy or Speech Therapy.
Payment options include:

* Private Pay (personal check, visa, mastercard, discover card)
These services are generally not covered by Medicaid.
Nursing Care
(Nursing Care, Private Duty Nursing & Psychiatric Nursing)
Nursing Care Services are provided by Registered Nurses, Licensed Practical Nurses and Certified Nursing Assistants. All nursing care is provided under the guidance of the patient's physician. Patients meeting at least tow of the conditions listed below may qualify for nursing care services.

Referrals for Nursing Care are made by the patient's physician. When Doctors Home Care receives the referral, a Registered Nurse Supervisor will go to the patient's home (or the hospital) for an evaluation and to begin the admission process. Scheduling of in-home services can then begin. The RN supervisor and nursing team work with the physician to develop a plan of treatment and to conduct paitent evaluations in order eto ensure the best quality of care for each patient.
