OCMC's home and community based health care division provides appropriate and effective physician directed care to individuals in their own home. Home care provides relief for family members who give continuous care to patients with long-term illnesses. Services provided include:

Dietary Counseling
 
Dietary Counseling is available for individuals whose medical condition warrants a special diet. Following a physician referral, the dietitian assesses the patients current eating habits and interprets the physicians order into a workable meal plan. The dietitian also offers advice to caregivers on shopping for and preparing proper nutritional choices for the patient.
 
ElderChoices
 
ElderChoices is for individuals who due to physical, cognitive or medical reasons, require a level of assistance that would have to be provided in a nursing facility, if it were not for the services offered through this program. The following services are available to the elderly:

In addition to ElderChoices services, waiver recipients may receive other Medicaid covered services such as physician visits, some prescription drugs and personal care services.

Extended Care Services
 
Extended Care is for the patient who requires six or more hours of continuous home health service in a 24-hour period. Extended care is staffed by Registered Nurses, Licensed Practical Nurses and Certified Nursing Assistants. All nursing care is provided under the guidance of the patient's physician. Patients requiring the following services are eligible:

* Parenteral Nutrition
 
* Prolonged IV Drug Therapy
 
* Ventilator
* Oxygen Supplementation
 
* Peritoneal Dialysis
 
* Tube Feeding (gastrostomy,
 
naso or gastric feeding)

Referrals for extended care are made by the patient's physician. When Doctors Home Care receives the referral, a Registered Nurse will go to the patient's home (or the hospital) for an evaluation. Once the in-home services begin the supervisor and nursing team work with the physician to develop a plan of treatment, to review a medication regimen and to conduct patient evaluations in order to ensure the best quality of care for each extended care patient.
